Key Responsibilities\n

• Conduct comprehensive mental health assessments, thoroughly evaluating patients’ mental health status in conjunction with their medical history and current medical conditions.

• Develop and implement integrated treatment plans that address psychiatric diagnoses as well as any underlying medical issues, ensuring an inclusive approach to patient care.

• Facilitate individual and group therapy sessions using evidence-based practices tailored specifically to the needs of each patient, fostering a safe and supportive therapeutic environment.

• Work in close collaboration with medical providers to ensure cohesive care models and effective treatment outcomes, thereby addressing the physical and mental health needs of patients.

• Provide crisis intervention services and support to patients experiencing acute mental health challenges, employing de-escalation techniques and appropriate referral processes.

• Educate patients on the importance of mental health in relation to physical health, equipping them with coping strategies and management skills to empower self-care.

• Maintain meticulous and confidential documentation in accordance with legal, ethical, and organizational standards, ensuring that all patient information is secure and up-to-date.

• Participate in ongoing professional development, staying informed of emerging research and best practices in the mental health field and integrated care methodologies.

Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Active and valid licensure as an LCSW, LPCC, or LMFT in the state of California required.

• Demonstrated experience working with clients exhibiting co-occurring medical and mental health needs.

• Exceptional interpersonal and communication skills, with the ability to establish trust and rapport with clients and colleagues.

• Strong understanding of professional ethics and the capability to maintain confidentiality.

• Proficiency in Microsoft Office and familiarity with electronic health record systems.
