Job Summary:

The Medi-CAL Coordinator is responsible to screen individuals for placement based on their Medi-CAL eligibility and coverage for services that will address their criminogenic needs, including substance use disorders. The intention of our services is to help people who have not been able to get the needed help in terms of Housing, Substance Abuse treatment, co-occurring disorders, and linkages to community resources to help minimize recidivism. This involves the coordination of medical, behavioral, and social services to address the full range of student/participant needs, and, identifying and referring those with Severe Medical and/or Mental Health conditions needing a higher level of care via state and local funding.
The Medi-CAL Coordinator will work out of the STOP Placement Area 5 Office, CalAIM Enhanced Case Management (ECM) office, and Parole Offices in Los Angeles County. This position assists the Reentry Assessment Specialists, Reentry Assessment Coordinator, and Director of Operations with Medi-CAL eligibility determination and enter into the California Department of Corrections and Rehabilitation (CDCR) Automated Reentry Management System (ARMS) Database. This will include both Drug Medi-CAL enrollments and STOP placement recommendations (as needed on a case-by-case basis). This position may be inadvertently updated based on changing State contractual requirements.

Essential Duties:

•Conduct screening and determine eligibility for Medi-CAL services for STOP referrals, with an emphasis on criminogenic risks and needs and substance use and related factors.

•Generate tailored placement recommendations based on Medi-CAL screening eligibility, assessment results, aligning individuals with suitable STOP and/or Medi-CAL program modalities (as needed).

•Consistent utilization of knowledge of Medi-CAL, Drug Medi-CAL (DMC), and CalAIM systems of care and enrollment requirements.

•California Department of Corrections and Rehabilitation (CDCR) Automated Reentry Management System (ARMS) Database reception entries of Medi-CAL services eligibility upon STOP referral/enrollment.

•Conduct outreach to identify and engage eligible for CalAIM ECM services, using multiple strategies and modalities for engagement.

•Collaborate with parole and program staff to coordinate referrals and ensure timely, appropriate placements.

•Assist with placement logistics and support, verifying program capacity and participant readiness based on screening and/or assessment outcomes.

•Coordinate care across multiple providers and services, ensuring seamless integration of medical and social care.

•Upon receipt of a FACE Sheet and a 1502 Referral Form or PVD Form (for lifers) from Parole, the results of prior assessments and evaluative information, the Medi-CAL Coordinator will help coordinate an appropriate STOP Modality placement in conjunction with the Reentry Assessment Coordinator and/or Specialists as needed.

•All placement requests are validated with the Data Coordinator prior to participants enrollment in any program to ensure that services are authorized and will be paid for services.

•Ensures all required documentation is entered in ARMS to support enrollment.

•Strong collaboration and coordination with Parole Offices, In-Prison Community Transition Program (CTP) Parole Service Associates (PSAs) and Parole Agents to ensure appropriate and approved placements.

•Delegates referrals to ensure an appropriate workload distribution in collaboration with the Reentry Assessment Coordinator and/or Director of Operations as needed.

•Reports to senior leadership any issues regarding participant placement, requests for placement, or procedural issues relating to placement which need additional support.

•Co-Facilitate regular team meetings with the Reentry Assessment Coordinator and/or Director of Operations to review patient cases, address challenges, and share successes.

•Supervises the work of one (1) Reentry Assessment Specialist or one (1) Admin Assistant.

•Performs other related duties as assigned.

MINIMUM QUALIFICATIONS & COMPETENCIES

Education and Experience:
Required:

•Bachelor’s degree in social work, psychology, public health, or related field; or Associate’s degree

•And minimum of two years’ experience in client-related field, providing case management and community outreach.

OR

•The equivalent of a high school diploma and three cumulative (3) years of experience of administrative responsibility in providing outreach, substance abuse counseling, reentry services, and case management within a culturally diverse population.

•Working knowledge of Medi-CAL, Drug Medi-CAL (DMC), and CalAIM systems of care and enrollment requirements.

•Strong communication and interpersonal skills, capable of building relationships with students, families, team members, and community partners.

•Understand various ethnic and socioeconomic backgrounds within a multicultural environment.

•Knowledge on how to administer screenings and assessments, substance abuse counseling, reentry services, and case management within a culturally diverse population, preferably with both men and women.

Preferred:

•Experience in administering assessments with a focus on criminogenic needs and/or substance use disorders

•Bilingual, fluent Spanish/English oral and written skills are highly desirable.

•As a TACPA Worksite, priority consideration is given to qualified individuals as specified in GC 4530.

•Priority consideration is given to recipients of aid under Welfare and Institutions Code Section 11200.

Certifications or Licenses:

Required:

•Must possess a valid California driver’s license and Auto Insurance

•Must satisfy requirements for CDCR Clearance

Preferred:

•It is desired, but not mandatory, to possess a Substance Use Disorder Certified Counselor Credential (SUDCCC) from an accredited certifying organization.

Knowledge:

•Understanding of causes, symptoms, and effects of substance abuse.

•Understand an array of treatment models available to participants and maintain a current awareness of availability for

placement within each treatment paradigm.

•Recognition of the importance of individual differences created by personalities, cultures, lifestyles, social networks, and other factors that influence behavior. Demonstrate the ability to apply this knowledge to treatment practices.

Skills/Abilities:

•Excellent interpersonal and communications skills and the ability to work effectively with a wide range of constituencies in the community and within the organization.

•Ability to integrate the results of participant assessments in placement recommendations.

•Ability to integrate and align treatment options with institutional recommendations and clinical evaluation results.

•Ability to work constructively with diverse people and with parties that may have divergent perspectives and interests.

•Ensure that excellence and integrity are applied to professional responsibilities to provide optimal services to participants.

•Understand and appropriately apply Amity policies and procedures and adhere to agencywide practices and regulations.

•Protect program participants’ health, safety, welfare, and confidentiality of records and participant information.

•Model professionalism, effective work habits and responsible living. 

Training and Continuing Education

•Within 90 days of hire, personnel file shall document evidence of participation in the following training:

• Employee Orientation
• CPR Certification, First Aid Certification (within 6 weeks of hire, and renewed prior to expiration thereafter)
  Tuberculosis (TB) test (annual)
• Motivational Interviewing
• Sexual Harassment Prevention
• Emergency Procedures
• Confidentiality (Title 42 Code of Federal Regulations (CFR) Part 2)
• Health Insurance Portability Accountability Act (Title 45 CFR Part 164)
• Signed acknowledgement of Drug Free Workplace policyo
• Signed CDCR Prison Rape Elimination Policy (PREA) form

•Participate in forty (40) hours of applicable orientation training within the first year of employment, and forty (40) hours annually thereafter. Training topics will be consistent with the duties and responsibilities of the position and applicable to the function of the STOP Placement Office.

•Participate in other training as assigned.

Security Clearance & Live Scan

•This position shall go through the Provisional and Security Clearance Processes defined by California Department of Corrections and Rehabilitation (CDCR), including Background Security Clearance Application and Live Scan;

•Must receive and maintain security clearance.
