Work Environment

This position offers an onsite work environment within the setting of home care. You will travel to patient residences in the Chantilly area, providing individualized therapy in the comfort of their homes. Key Responsibilities

• Conduct thorough examinations and evaluations to develop customized treatment plans for patients experiencing functional limitations and disabilities.
• Provide expert consultation to other healthcare professionals regarding the continuum of care for patients, ensuring continuity and effectiveness in treatment.
• Engage with patients of varying ages and cultural backgrounds, delivering exceptional customer service and fostering a supportive atmosphere.
• Implement treatment plans effectively while adhering to professional standards and documentation protocols.
• Evaluate patient progress regularly and adjust treatment plans as necessary, ensuring compliance with applicable laws and regulations.
• Maintain open communication and collaboration with interdisciplinary teams, including referral sources and physicians, to optimize patient outcomes.
• Participate in community outreach programs, helping to raise awareness of physical therapy services while developing valuable programs and educational opportunities.

Patient Population / Client Focus

The Physical Therapist will primarily work with individuals who have diverse physical needs related to rehabilitation from injury, surgery, or chronic health conditions. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensing in Physical Therapy and Basic Life Support (BLS) certification are mandatory.

Compensation and Benefits
