Work Environment

This role is onsite and involves delivering care directly in patients' homes, allowing you to build meaningful relationships with your patients while providing therapy in a comfortable, familiar setting. Key Responsibilities

• Deliver high-quality, compassionate, and safe physical therapy services to patients in their homes, ensuring that treatment aligns with established clinical guidelines and best practices.
• Develop and implement individualized treatment programs based on comprehensive evaluations and the specific goals established by the physical therapist.
• Conduct thorough evaluations, taking detailed patient histories, performing clinical assessments, and documenting findings to form actionable recommendations.
• Prepare and execute effective discharge plans, providing thorough follow-up care and patient education to facilitate ongoing recovery.
• Document all patient interactions meticulously, including initial evaluations, daily notes, interim progress summaries, and discharge evaluations, adhering to compliance standards in health care documentation.
• Stay current with corporate and clinical training requirements, actively participating in ongoing education to maintain licensure and compliance with state and federal regulations.
• Collaborate with healthcare providers, family members, and third-party payers through clear and respectful communication to ensure comprehensive care for each patient.
• Engage in facility meetings and case consultations as directed by management, contributing to continuous improvement discussions.

Patient Population / Client Focus

The patient population you will serve primarily consists of individuals recovering from orthopedic conditions, including post-surgical candidates such as joint replacement or fracture repair, as well as patients with chronic pain and mobility challenges. You will play a pivotal role in helping these patients regain strength, mobility, and independence, directly impacting their overall health and well-being.

Qualifications

• Must be a graduate of an accredited Doctor of Physical Therapy program.
• A valid state license to practice physical therapy in Virginia is required.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandatory.
• Excellent interpersonal and communication skills, with the ability to work effectively in a team-oriented environment.

Preferred Experience

• Minimum of two years of clinical experience in physical therapy, ideally including home care or acute care settings.
• Experience with orthopedic rehabilitation and patient-centered treatment planning is highly advantageous.
