Work Environment

Key Responsibilities

• Conduct thorough evaluations, including a review of pertinent medical history, specific assessments, and documentation of results, to develop tailored treatment plans.
• Communicate evaluation findings and treatment strategies effectively to patients and families, actively involving them in the goal-setting process to ensure alignment with their preferences and needs.
• Implement evidence-based treatment protocols that reflect the findings of your evaluations, adapting to the patient's ongoing progress and the evolving healthcare environment.
• Consult with other healthcare providers as needed to refine patient care plans, ensuring that modifications are documented in response to any changes in patient condition.
• Manage discharge planning and follow-up care diligently, ensuring accurate and timely documentation of all patient interactions and progress reports.

Patient Population / Client Focus

Qualifications
