Key Responsibilities\n

• Manage a designated caseload of patients and conduct thorough evaluations to assess the social, emotional, financial, and cultural factors affecting their care. Utilize a systematic, evidence-based approach in deploying various therapeutic modalities to clear psychosocial barriers.

• Lead healthcare team coordination in tackling protective service issues, executing life care decisions, and establishing advance directives for patients.
\n
• Develop and implement comprehensive treatment plans addressing complex psychosocial issues, particularly for high-risk patients, and provide crisis intervention as necessary.
\n
• Collaborate with case managers and interdisciplinary team members to ensure patients' psychosocial and socioeconomic needs are fully integrated into their treatment and discharge plans.
\n
• Supervise and mentor social work staff, MSWs, interns, and volunteers, facilitating weekly supervision while participating in the establishment of effective practice standards.
\n
• Educate patients, families, and community agencies on resource utilization, preventing readmission, and ensuring a smooth transition through services both public and private.
\n
• Respond promptly to psychosocial service requests, ensuring timely implementation of treatment plans and effective patient follow-through of care responsibilities.
\n
• Remain engaged with the healthcare team, attending specialty rounds, patient care conferences, and ensuring continuity of care by maintaining communications with external community agencies.

• Ability to support colleagues and patients on-site when necessary, offering crisis support and interventions as required.
\n
• Fulfill on-call responsibilities as required by the Department Director to ensure support during critical times.

Qualifications\n

• Master’s Degree in Social Work (MSW) is required.

• Current and valid California LCSW license is mandatory.

• Minimum of 2 years of experience working as a Licensed Clinical Social Worker, preferably in a healthcare setting.

• Strong communication, problem-solving, and interpersonal skills are essential for effective collaboration within a multidisciplinary team.

Preferred Experience\n

• Experience working in hospice, palliative care, or with a geriatric population is highly desirable.

Compensation and Benefits\n

We offer a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience. In addition, we provide a comprehensive benefits package that includes health insurance, retirement plans, paid time off, and opportunities for continuing education.
