As part of a hospice center’s interdisciplinary team, you will work closely with healthcare professionals, including physicians, nurses, and spiritual care providers, to ensure holistic support for our clients and their families.

Key Responsibilities\n

• Develop and implement individualized support plans that align with each client's unique needs, working towards measurable outcomes.

• Conduct comprehensive interviews with service users to assess their current conditions, identifying their strengths and areas for improvement.

• Utilize evidence-based practices to set and evaluate realistic therapeutic goals tailored to each client’s situation.
\n
• Continuously monitor client progress, adapting treatment plans as necessary to reflect changes in needs or circumstances.

• Provide direct counseling services, offering insightful guidance and support on a variety of emotional and social issues encountered by clients.
\n
• Maintain meticulous documentation of client interactions, progress notes, and treatment plans to ensure compliance and facilitate communication within the care team.

• Participate in case conferences, offering insights and recommendations based on your expertise, and may provide testimony in court as needed.
\n
• Act as a key-worker, collaborating with other professionals to coordinate care and refer clients to community support services that assist in their recovery efforts.

Patient Population / Client Focus\n

In this role, you will primarily work with clients at various stages of life facing complex end-of-life issues, such as terminal illnesses, grief, and emotional distress. Your ability to empathize and connect with individuals from diverse backgrounds will be crucial in supporting their emotional, psychological, and social needs during vulnerable times.

Qualifications\n

• A Master’s degree in Social Work (MSW), Psychology, or Sociology is required, along with active licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated professional experience as a social worker, preferably within a healthcare or hospice setting.

• Knowledge of social theories and practices relevant to mental health and clinical social work.
\n
• Strong interpersonal skills, including social perceptiveness, empathy, and the ability to build therapeutic relationships.

• Resilience and adaptability, enabling effective assessment and management of various client scenarios.

• Willingness to undergo a background check for safeguarding purposes.

Compensation and Benefits\n
