Key Responsibilities\n

• Conduct comprehensive assessments to identify the emotional, social, and environmental needs of clients and their families facing end-of-life challenges.

• Develop and implement personalized care plans that outline specific goals, interventions, and methods for tracking progress throughout the hospice journey.

• Provide compassionate counseling and supportive services to individuals and families, facilitating open conversations about grief, loss, and transition.

• Collate case notes and maintain electronic health records using relevant social work systems to ensure accuracy in client documentation.

• Perform crisis intervention and risk assessments as necessary, providing immediate support to clients in times of emotional distress.

• Collaborate closely with healthcare providers, community organizations, and family members to coordinate comprehensive care and services for clients.

• Engage in proactive community outreach initiatives that promote awareness of hospice services and available resources, helping to destigmatize discussions around end-of-life care.

Patient Population / Client Focus\n

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• An active state license as a Licensed Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n
