Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to the individual needs of clients, addressing mental health, substance abuse, and medical requirements within the primary care framework.

• Provide a variety of clinical services including individual therapy, group counseling, crisis intervention, and treatment planning that effectively support client goals and recovery.

• Develop and document mental health evaluations, progress notes, and treatment plans in accordance with best practices and regulatory standards, utilizing electronic health record systems.

• Collaborate with healthcare providers to escalate patient needs, ensuring timely referrals to additional services or specialists when necessary.

• Facilitate access to community resources and support services, including medication assistance programs, to help clients navigate their care options.
\n
• Participate in quality improvement initiatives to enhance service delivery, actively contributing to data collection and analysis efforts.

Patient Population / Client Focus\n

The primary focus of this role includes working with individuals, couples, and families who present with mental health challenges, substance use issues, and medical-related social determinants of health. Our clientele often includes marginalized populations, which requires a culturally sensitive approach and an openness to understanding diverse backgrounds and experiences.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.

• Minimum of three (3) years of clinical experience following licensure, preferably in a mental health or clinical setting.

• Demonstrated knowledge of various counseling techniques and crisis intervention practices, with the capability to exercise independent judgment in clinical decision-making.

• Effective communication and interpersonal skills to foster trusting relationships with clients and colleagues alike.

Preferred Experience\n

• Two years or more experience working in a behavioral health setting or community mental health environment.

Compensation and Benefits\n
