Key Responsibilities\n

• Conduct comprehensive assessments with clients to establish their goals, preferences, and available treatment options, ensuring their voiced needs are prioritized.

• Design and implement individualized rehabilitation plans tailored to clients' education levels, skills, and aspirations, regularly updating these plans as circumstances change.

• Collaborate closely with a diverse team of healthcare providers to formulate and execute effective rehabilitation programs aimed at facilitating client progress and wellness.

• Maintain detailed case files for each client, documenting personal information, eligibility criteria, service histories, and findings from ongoing evaluations.

• Monitor client progress meticulously, making adjustments to therapy as needed to ensure that therapeutic goals are met efficiently.

• Engage regularly with clients during job training, occupational programs, and lifestyle adjustments, addressing and resolving challenges as they arise.

• Involve family members and significant others in the treatment process to ensure a well-rounded support system for clients.

• Produce and submit timely reports on client needs and progress, providing recommendations for additional resources or referrals when necessary.

As a Licensed Professional Counselor at Millennia, you will work with a broad spectrum of clients, including individuals dealing with mental health and mood disorders. Qualifications\n
