Key Responsibilities\n

• Conduct comprehensive assessments for clients, utilizing standardized tools and interviews to formulate accurate diagnoses and tailor treatment plans that meet their unique needs.

• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and solution-focused therapy, to foster client engagement and progress.

• Maintain meticulous clinical records, ensuring all documentation is completed on time, accurate, and compliant with strict confidentiality laws and regulations.

• Collaborate closely with a multidisciplinary treatment team, including psychiatrists, nurses, and case managers, to ensure a holistic approach to client care.

Patient Population / Client Focus\n

In this role, you will work primarily with clients in hospice care, addressing their psychological needs as they manage end-of-life issues and preparing for transitions. This includes supporting patients with conditions such as depression, anxiety, grief, and adjustment disorders, ensuring they receive the empathetic and skilled support necessary during such a critical time in their lives.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current and active Licensed Clinical Social Worker (LCSW) license in the state of Arizona, with no history of disciplinary actions.

Compensation and Benefits\n
