Work Environment\n

This position is fully remote, allowing you to provide care and support from the comfort of your own home while maintaining flexibility to meet client needs. Your primary interactions will be through telehealth services, ensuring ongoing communication and support is available for both clients and their families. You will collaborate with a multidisciplinary team of healthcare professionals, including nurses, therapists, and physicians, to ensure holistic care delivery and address any challenges that arise in client care.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors to identify their emotional and social needs, while using evidence-based practices to inform treatment plans.

• Develop and implement personalized care plans in collaboration with clients and their families, focusing on enhancing their overall well-being and meeting their specific goals.

• Facilitate individual and family counseling sessions via teletherapy, providing emotional support and guidance during challenging life transitions.

• Advocate for clients’ rights, assisting them in accessing essential community resources, support services, and benefits that promote their quality of life.

• Maintain a close partnership with multidisciplinary teams, regularly communicating client needs and concerns to ensure cohesive care delivery.

• Conduct ongoing monitoring of clients through virtual visits to assess their living conditions, adapt care plans as needed, and ensure safety and comfort.

• Educate clients and families about available resources, support groups, and programs aimed at enhancing social engagement and emotional well-being.

• Ensure compliance with all documentation standards by maintaining accurate records of client assessments, care plans, and progress notes in the electronic health record (EHR) system.

• Stay updated on best practice guidelines and state regulations regarding social work services and hospice care.

Patient Population / Client Focus\n

In this role, you will primarily serve seniors who are in hospice care, addressing a range of psychosocial issues related to aging, terminal illness, and end-of-life decisions. Your ability to empathize and connect with clients and their families will be crucial in fostering trust and providing the necessary support during a vulnerable phase of life.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW).

• At least two years of experience in a social work position, preferably in hospice, geriatric care, or a community-based setting.

• Solid understanding of aging-related issues, social work principles, and knowledge of community resources available to seniors.

• Exceptional communication, assessment, and problem-solving skills.

• Demonstrated ability to empathize with and build rapport with seniors and their families during periods of significant emotional distress.
