Key Responsibilities\n

• Conduct comprehensive psychosocial assessments with patients and families to identify emotional, social, and environmental factors affecting their care.

• Develop and implement personalized treatment plans that address individual patient needs, incorporating therapeutic interventions and supportive measures.

• Provide individual and family therapy using evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care.

• Offer crisis intervention and solutions to manage immediate psychological distress while facilitating referrals to community resources.

• Collaborate with a multidisciplinary team, including medical and nursing staff, to coordinate comprehensive care and address complex patient needs.

• Maintain accurate and timely documentation of patient interactions and treatment plans, ensuring compliance with state regulations and organizational policies.

Qualifications\n

• Active Licensed Clinical Social Worker (LCSW) license in New York, in good standing.
