Key Responsibilities\n

• Conduct thorough assessments of clients via telephone and in-person to gather relevant historical and current information for effective case management.

• Develop individualized treatment plans based on comprehensive assessments, targeting specific needs and therapeutic goals.

• Provide immediate crisis intervention as required, employing strong de-escalation strategies to mitigate emotional distress in clients.

• Actively participate in case coordination by contacting and making referrals to other agencies and supportive services to enhance client outcomes.

• Engage clients in therapeutic activities designed to promote recovery and well-being, ensuring they remain engaged with available resources.

• Maintain up-to-date and accurate documentation of client interactions, assessments, and treatment plans in compliance with organizational policies and regulations.

• Monitor and remain vigilant for signs of child abuse and other critical factors, intervening as necessary to protect and support vulnerable clients.

Qualifications\n
