Key Responsibilities\n

• Conduct thorough and ongoing assessments to determine the individual needs of patients, adapting treatment plans as necessary.

• Foster open lines of communication among supervisors, other mental health professionals, families, and guardians regarding each client's progress and needs.

• Develop and present a tentative appointment schedule that includes home visits and necessary clinical observations to supervisors.

• Coordinate and execute home visits for comprehensive assessments that include chart reviews and interviews to ensure direct observational feedback.
\n
• Implement and oversee the effectiveness of treatment programs, ensuring adherence to individualized patient care strategies.
\n
• Actively participate in Interdisciplinary Team Meetings, sharing insights and collaborating on the delivery of effective mental health treatment.

• Complete detailed monthly and quarterly progress notes to maintain accurate documentation of patient growth and treatment outcomes.

• Organize training sessions to ensure all staff members are well-versed in the program plan and documentation requirements.
\n
• Provide ongoing support through follow-up interviews and observations, reinforcing the treatment process for each client.

• Maintain meticulous records to ensure that critical information is accessible to relevant stakeholders.

• Serve as a liaison to the broader community and mental health professionals, aiding in program development and fostering partnerships.

• Contribute to the creation and refinement of training procedures and materials aimed at enhancing program effectiveness.
