Work Environment\n

This position allows for remote work flexibility while interfacing with clients primarily in a hospice center. The hours are typically Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs and company demands. On-call availability may be needed to ensure continuity of care, demonstrating our commitment to our clients’ well-being.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, integrating diverse emotional, mental, and social factors that affect their overall health.

• Collaborate closely with seniors and their families to create personalized care plans that align with their specific goals, values, and preferences.

• Provide counseling and emotional support tailored to the unique challenges faced by seniors in hospice, ensuring they feel heard and valued.

• Advocate for clients to ensure access to essential community resources, services, and support systems, addressing barriers to care.

• Work collaboratively with other healthcare professionals, including nurses and caregivers, to coordinate comprehensive care and service delivery.

• Monitor clients through regular visits to assess their living conditions, help manage symptoms, and ensure safety and comfort.

• Facilitate healthy communication among family members involved in the care process, helping to resolve conflicts and educate them on caregiving roles.

• Maintain thorough documentation of client interactions, care plans, and progress notes as part of compliance with regulatory standards.

• Stay current on best practices and evolving policies in social work and hospice care to ensure ethical and high-quality service delivery.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is essential.

• A minimum of two years of direct experience in social work, specifically in a community-based or geriatric setting, showcases your ability to connect with our population.

• In-depth knowledge of social work techniques, especially those tailored to the elderly and palliative care.

• Strong assessment, counseling, and crisis intervention skills, enabling you to engage effectively with both clients and their families.

Compensation and Benefits\n

The salary for this position ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications.
