Key Responsibilities

• Direct and oversee comprehensive physical therapy interventions designed to prevent disabilities, alleviate discomfort, enhance muscle function, and optimize patient performance in adherence to established care plans.
• Conduct initial evaluations to diagnose and assess patients' physical capabilities through a variety of tests focused on muscle, nerve, joint function, and daily life activities.
• Formulate individualized treatment plans that incorporate evidence-based practices, ensuring compliance with physician prescriptions.
• Provide hands-on treatment, including therapeutic exercises, manual therapy techniques, and modalities such as traction and massage.
• Educate patients and their families on rehabilitation strategies, self-management techniques, and the use of assistive devices to support independence.
• Monitor patient progress diligently; document clinical findings and treatment responses accurately in clinical records, including OASIS reports, care plans, and discharge summaries.
• Lead and mentor physical therapy assistants, ensuring the appropriate execution of care plans while meeting supervision requirements dictated by state regulations.
• Participate actively in case conferences and collaborative meetings to discuss treatment plans and patient progress across the interdisciplinary team.
• Make home environment assessments to suggest modifications that would facilitate optimal patient independence during recovery.
• Adhere to all compliance standards set forth by federal, state, and agency regulations while maintaining high standards of patient confidentiality.
