Home Health RN Case Manager

Responsibilities Provide skilled nursing care to patients in their homes Perform comprehensive patient assessments and develop individualized plans of care Monitor patient progress and adjust care plans as needed Educate patients, families, and caregivers on disease management, medications, treatments, and safety practices Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate care Complete timely and accurate clinical documentation Identify changes in patient conditions and communicate findings to the care team Participate in case conferences and interdisciplinary care planning Ensure compliance with all regulatory, agency, and professional nursing standards Maintain patient confidentiality and HIPAA compliance at all times Requirements Current, unrestricted Registered Nurse (RN) license in the state of practice Minimum of one (1) year of nursing experience in a clinical care setting Strong assessment, critical thinking, and communication skills Ability to work independently and effectively manage a patient caseload Reliable transportation, a valid driver's license, and current auto insurance Willingness to travel throughout the assigned service area Physical Requirements: Sit, stand, bend, lift, and move intermittently, and be able to lift 50-100 lbs Preferred Qualifications: Home Health experience, Hospice experience, Experience with HomeCare HomeBase (HCHB), Associate's or Bachelor's Degree in Nursing #J-18808-Ljbffr Jobtailor
