RESPONSIBILITIES

Under direction of the Program Manager, provides overall service coordination to assigned clients; participates in the assessment of the needs of persons with developmental disabilities; acts as team leader and facilitates the development, implementation and coordination of an Individual Program Plan (IPP)/Individual Family Service Plan (IFSP); assists in locating and securing services and supports; acts as an advocate; evaluates and monitors the quality and effectiveness of services.

QUALIFICATIONS

EDUCATION/EXPERIENCE Bachelor’s Degree from an accredited university, preferably in a Human Services field and 1 year related experience which must be in the field of developmental disabilities for non‑Human Services majors; or have a Master’s Degree from an accredited university in Social Work, Psychology, Special Education, or a related field.

QUALIFICATIONS

RESPONSIBILITIES

Under the direction of the Community Placement Program (CPP) Program Manager, the CPP Nurse supports individuals with complex needs transitioning from secure facilities into community settings. These individuals may be involved with multiple systems and present with a range of intellectual, developmental, mental health, and medical conditions. The CPP Nurse conducts clinical assessments, performs quality‑assurance visits to CPP residential homes, and identifies the healthcare and developmental needs of those served. The role also provides clinical guidance to the CPP team in developing health‑related goals for Individual Program Plans and assists vendors in creating Restrictive Health Care Plans.

QUALIFICATIONS

EDUCATION Graduation from a school of nursing approved by the National League for Nursing; a Bachelor’s degree in Nursing is preferred. A valid license as a Registered Nurse in the State of California is required. EXPERIENCE Two (2) years of Nursing experience in a health care setting. Experience in working with individuals with a developmental disability is preferred. OTHER Ability to solve problems with minimal supervision; function within an Interdisciplinary or Planning Team setting; prepare clear and concise reports. Knowledge of normal and abnormal growth and development; effect of cultural and social patterns on health; organization and functions of social, health and other support service agencies; current nursing practice methods and procedures; and state and federal legislation related to service for the developmentally disabled.

Under the direction and supervision of the Coordinator, Psychology Services, the position provides professional psychological consultation regarding diagnostic assessment, eligibility determination, and the ongoing support of individuals with developmental disabilities. The role collaborates with interdisciplinary staff and supports service planning and decision‑making to promote timely, high‑quality psychological services.

QUALIFICATIONS

EDUCATION Doctoral degree (Ph.D./Psy.D.) in Psychology from an APA‑accredited university. Licensed by the California Board of Psychology (BOP) as a Psychologist. EXPERIENCE Must have at least three (3) years of experience in a clinic, hospital, or public agency. A significant portion of this time should have been spent in the evaluation and/or treatment of people with developmental disabilities. Must have experience in functioning as a part of a multi‑disciplinary team. OTHER Ability to interpret and complete psychological/developmental evaluations with children and across the lifespan; communicate effectively with co‑workers, parents and clients verbally and in writing; relate well to representatives of the community and associated agencies; work effectively in an interdisciplinary framework. Must be familiar with a wide variety of psychological tests. Required to have a specialized knowledge of developmental psychology, tests and measurement as well as psychological procedures with a focus on Autism and Intellectual Disabilities. Understanding of DDS policies, and regulations preferred.
