Work Environment\n

This role is remote, allowing you the flexibility to manage your work schedule while still engaging closely with clients. Although your primary duties will be conducted virtually, you may occasionally visit our Hospice Center in Elgin or attend team meetings as needed. Our operational hours are Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs. This setting fosters collaboration among clinicians and ensures comprehensive care delivery.

Key Responsibilities\n

• Provide therapeutic services directly as a licensed clinician to assigned clients, utilizing evidence-based practices to address mental health conditions and emotional disorders.

• Conduct thorough assessments, developing treatment plans tailored to the unique needs of each client, and consistently monitor their progress towards established goals.

• Collaborate effectively with a multidisciplinary team to enhance client outcomes, demonstrating a commitment to teamwork and communication in achieving shared objectives.

• Engage respectfully with clients, families, and colleagues, recognizing and valuing diverse backgrounds, beliefs, and experiences while maintaining professional conduct.

• Navigate the organizational and financial structures of the center, facilitating appropriate referrals and resource allocation to optimize service delivery.

• Adhere to professional ethical guidelines and maintain confidentiality in accordance with regulatory standards and the policies of Conscious Minds.

Patient Population / Client Focus\n

In this role, you will primarily work with clients who are experiencing symptoms of major mental illness and severe emotional disorders. Our diverse client base includes individuals from various backgrounds, ages, and socio-economic statuses. As a key player in our holistic treatment approach, you will facilitate access to essential mental health resources and support, ultimately contributing to improved wellness and community resilience.

Qualifications\n

• A Bachelor's degree in Social Work, Psychology, or a related field, with an active LCSW license in Illinois.
