Key Responsibilities\n

• Effectively communicate patient progress, needs, and concerns to other healthcare providers, ensuring cohesive care and timely interventions.

• Maintain precise and timely clinical documentation, including progress notes in accordance with departmental policies and legal regulations.

• Collaborate with the healthcare team to develop comprehensive treatment plans, including thorough psychosocial assessments and discharge planning using accepted social work interventions.

• Provide counseling and crisis intervention to clients, skillfully assessing emotional and behavioral issues while implementing appropriate therapeutic strategies.

• Forge and nurture relationships with community resource groups, representing Holistic Recovery Counselling in meetings and committees to advocate for patients' social needs and improve resource accessibility.

Patient Population / Client Focus\n

You will be working with a varied patient population, including individuals facing mental health challenges such as anxiety, depression, substance use disorders, and trauma-related issues. Qualifications\n

• A Master’s degree in Social Work from an accredited institution, with a current state license as a Licensed Clinical Social Worker (LCSW) along with any pending details required by the Board of Behavioral Sciences.

• Strong organizational skills and a proven ability to manage caseloads while meeting necessary documentation deadlines.
