Key Responsibilities\n

• Plan, coordinate, and implement tailored support packages for clients dealing with a variety of challenges, including mental health disorders, family issues, and substance dependencies.

• Conduct comprehensive intake interviews to assess clients' current circumstances, needs, strengths, and weaknesses, ensuring a thorough understanding of individual situations.
\n
• Develop individualized treatment goals and objectives during case management sessions, focusing on measurable outcomes that lead to improvements in clients' lives.
\n
• Regularly monitor and evaluate clients' progress, making necessary adjustments to treatment plans based on their evolving needs and feedback.

• Provide valuable guidance during counseling sessions, advising clients on effective strategies and resources to assist in their recovery.

• Maintain and update accurate client records and documentation, ensuring compliance with relevant regulations and reporting requirements.

• Participate in case conferences to discuss clients' treatment and collaborate with fellow professionals, providing expert insights and recommendations.

• Act as a key-worker, facilitating access to additional services and resources within the community to further support clients’ recovery.

Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) or a related field is required; however, possession of a Master's Degree in Social Work (MSW) is preferred.

• A valid LCSW license to practice in the state of Texas.

• Proven experience working effectively as a Social Worker, preferably in a community health or counseling setting.

Compensation and Benefits\n
