Gotham Enterprises Ltd is on the lookout for a skilled and dedicated Nurse Practitioner (NP) to join our dynamic team within the Center for Advanced Cardiac Care (CACC) outpatient office located in Yonkers, New York. This full-time position allows for a collaborative working relationship with a diverse medical team to provide exceptional care to patients with cardiac conditions. The NP will be under the direction of the Chief Nurse Practitioner and the CACC Medical Director within our esteemed Division of Cardiology.

Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed histories and performing both comprehensive and targeted physical examinations.
• Order and interpret diagnostic tests, apply clinical judgment to manage both acute and chronic conditions effectively, and provide well-informed diagnosis.
• Develop and implement individualized care plans that encompass health maintenance, patient education, and follow-up strategies.
• Work collaboratively with cardiologists, referring physicians, and the multidisciplinary care team to optimize treatment plans tailored to patients' unique needs.
• Facilitate and coordinate hospital admissions when necessary, ensuring smooth transitions from outpatient to inpatient care.
• Engage in patient education and counseling, empowering patients and their families to understand treatment modalities, lifestyle modifications, and self-care strategies.
• Document all patient interactions and care plans in our electronic medical record system accurately and timely, contributing to continuity of care.
• Contribute to clinical research initiatives, applying findings to improve patient care protocols and engaging in discussions to foster an environment of continuous learning.
• Maintain a safe and therapeutic environment for both patients and staff, adhering to all regulatory and ethical standards in nurse practitioner practice.
• Take on additional responsibilities as required to meet evolving patient and organizational needs.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve adult patients dealing with various cardiovascular issues, including heart failure, pulmonary arterial hypertension, and post-heart transplant patients. This position requires an understanding of complex treatment protocols for patients undergoing therapies such as intravenous and inhaled medication support as well as mechanical circulatory support. The NP plays a vital role in providing high-quality care, fostering strong relationships with patients, and ensuring they receive the support they need throughout their healthcare journey.

Qualifications

• A Master’s degree in Nursing with Nurse Practitioner training focused on adult care.
• A current New York State Registered Professional Nurse and Nurse Practitioner license.
• Board certification as a Nurse Practitioner with up-to-date credentials in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), DEA, and NPI.
• Proficient in electronic medical records systems and familiar with MS Office Suite applications such as Word, PowerPoint, and Excel.

Preferred Experience

While a minimum of 2 years of clinical experience in a hospital setting is required, preference will be given to candidates with specific expertise in cardiology, particularly with patients facing advanced heart conditions and those requiring complex therapeutic interventions. Familiarity with multidisciplinary collaboration and experience in outpatient settings will also be advantageous.

Compensation and Benefits

The compensation for this position is competitive, ranging from USD 120,000 to 220,000 annually, commensurate with experience and qualifications.
