POSITION SUMMARY

• SUPERVISOR: Unit Manager

• P OSITION PURPOSE: The Adult Protective Service (APS) Risk Case Manager makes a difference in our community by supporting vulnerable adults to take actions to reduce the risk of harm.

  The APS Risk Case Manager works as a part of the APS unit to provide short-term intervention for older adults and adults with disabilities who are at high risk for abuse or neglect. The APS Risk Case Manager implements risk management plans to support stabilization and connections to resources.

  The APS Risk Case Manager relies on their expertise and knowledge of community resources while using a compassionate approach to positively affect people's lives.

  QUALIFICATIONS

  Minimum

  • High school diploma or GED and any combination of education and experience that provides the occupant with the desired skills, knowledge, and ability required to perform the job.
  • Bachelors' degree in social work or related field AND
  • Three years of progressively responsible experience working with seniors or people with disabilities
  • Basic computer skills and comprehension.
  • Must have a valid Oregon State driver's license.
  • Must pass a background check.
