Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to identify strengths and areas requiring support.

• Engage in the development and implementation of Individualized Family Service Plans (IFSPs), collaborating closely with families and multidisciplinary teams.

• Monitor child development outcomes, documenting progress and making adjustments to service delivery as necessary.
\n
• Provide hands-on support and resources to families, empowering them to enhance their child's developmental skills.
\n
• Ensure compliance with all federal and state regulations in assessment and intervention processes.
\n
• Utilize evidence-based practices in developing joint plans with families and recommend strategies in accordance with the Natural Learning Environment Practices.

• Foster strong, trusting relationships with children, families, and team members to create a healthy, supportive learning atmosphere.

• Maintain accurate documentation related to assessments, plans, and progress while adhering to all HIPAA regulations.

Patient Population / Client Focus\n

Our primary focus is on children from birth to age three who exhibit developmental delays or disabilities. The LCSW will work closely with families to promote skill development in areas such as physical, cognitive, social/emotional, and communication. Qualifications\n

• A Master's Degree in Social Work or a Doctorate in Social Work from an accredited institution.
