Work Environment

You will work onsite primarily in patients' homes, allowing for a flexible and dynamic work environment. Our team operates Monday through Friday, with a schedule designed to support both our therapists and our clients. Key Responsibilities

• Conduct thorough evaluations, including a review of patient history, performing specific assessments, and determining tailored treatment recommendations.
• Communicate evaluation findings and develop a comprehensive treatment plan collaboratively with patients and their families, ensuring their involvement in setting attainable goals.
• Implement treatment protocols based on your evaluations while adapting plans to fit each patient's evolving needs and the broader healthcare context.
• Consult with interdisciplinary team members when necessary, documenting changes in patient conditions and making informed decisions about care modifications.
• Manage discharge planning effectively, ensuring a smooth transition in care and facilitating follow-up procedures as needed.
• Maintain accurate documentation of patient interactions, including initial evaluations, daily progress notes, and discharge summaries, reflecting all stages of patient care.
• Diligently manage time and meet established productivity expectations while participating in community outreach and marketing activities promoting our services.
• Engage in ongoing professional development to enhance your skills and contribute to the goals set by our department.
• Encourage access to care by identifying barriers and actively participating in initiatives aimed at improving patient access to our services.

Patient Population / Client Focus

Your primary focus will be on patients recovering from various conditions that impact their mobility, strength, and overall physical well-being. This includes individuals rehabilitating from surgeries, injuries, or chronic illnesses. Our clientele often benefits from tailored interventions that address their specific needs, with an emphasis on building strong relationships to foster trust and collaboration throughout their recovery.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• Valid physical therapy license in the state of Virginia is necessary.
• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is a requirement.

Preferred Experience

• While prior work experience is not mandatory, two years in a physical therapy setting is preferred. This experience can include internships, clinical rotations, or directly related job roles.
• Experience in home care settings is valued, particularly in adapting therapy to the unique environments of patients' homes.

Compensation and Benefits

We are pleased to offer a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience, alongside a comprehensive benefits package. This includes:
