Work Environment

This position is based in a home care setting, requiring physical therapists to travel to patients' residences in the Centreville area. Key Responsibilities

• Conduct comprehensive evaluations of patients with functional limitations and disabilities, developing tailored treatment plans based on individual needs.
• Consult with healthcare providers to ensure coordinated and effective patient care throughout their recovery journey.
• Implement treatment strategies that promote patient engagement and motivation, including exercise regimens, manual therapy techniques, and patient education.
• Document evaluation findings, treatment plans, and progress notes accurately and timely in compliance with all applicable laws, rules, regulations, and professional standards.
• Collaborate effectively with patients, families, and other healthcare professionals throughout the treatment process, discussing any necessary adjustments to care plans with referring physicians.
• Engage in professional development activities, such as attending staff meetings, participating in continuing education programs, and staying current with best practices in physical therapy.

Qualifications

• Bachelor's Degree in Physical Therapy or a related field is required.
