Work Environment\n

As a Licensed Clinical Social Worker here, you will primarily work in a remote capacity, facilitating evaluations and family support while engaging with children and their families through telehealth channels. Our hospice center setting requires sensitivity and compassion, creating a warm and understanding atmosphere for our young clients and their families. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to identify developmental needs and appropriate interventions.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring all strategies are tailored to each child's unique circumstances.

• Regularly monitor and evaluate outcomes as part of a collaborative team, adjusting interventions based on continual assessment.

• Build and nurture positive relationships with children and families to establish a supportive and respectful learning environment.

• Provide families with relevant information, education, and coaching strategies that enhance their child's developmental progress.

• Implement evidence-based practices and Natural Learning Environment Practices in support of child-focused therapy.
\n
• Ensure compliance with all applicable Federal and State regulations, maintaining the highest standards of care for our clients.

Patient Population / Client Focus\n

The primary focus of this role is on children from birth to three years of age diagnosed with developmental disabilities or delays, particularly in a hospice care context. You will actively engage with these young clients, their families, and support networks to facilitate crucial early interventions that can dramatically affect their long-term development. By working closely with families, you will provide emotional as well as tactical support, ensuring they feel empowered throughout their journey.

Qualifications\n

• A Master's Degree or Doctorate in Social Work is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) awarded by the Department of Public Health is mandatory.

• A minimum of one year of experience in early intervention services, particularly with children is essential, demonstrating your capability in this specialized area.

Compensation and Benefits\n
