Key Responsibilities\n

• Conduct thorough evaluations and assessments to determine the developmental needs of infants and toddlers enrolled in our program.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSP) by collaborating with families and team members.

• Monitor developmental outcomes and adapt plans as necessary to ensure that children receive appropriate interventions based on their changing needs.

• Provide evidence-based support to families, equipping them with strategies to enhance their child's growth across all developmental domains.
\n
• Maintain compliance with all Federal and State regulations while implementing best practices in social work.
\n
• Work closely with families to develop actionable plans and recommend service frequency based on personal priorities and the child’s developmental status.

• Utilize natural environment teaching and a coaching model to empower families in promoting their children's development.

Patient Population / Client Focus\n

This position directly supports families with infants and toddlers facing developmental delays or disabilities. You will engage with children from diverse backgrounds, addressing their unique developmental challenges and providing individualized support that promotes growth in physical, cognitive, social-emotional, and communication skills. Qualifications\n

• A Master's Degree or Doctorate in Social Work (MSW/DSW) is required.

• Active Licensure as a Clinical Social Worker (LCSW) from the Department of Public Health is mandatory.

Compensation and Benefits\n
