This role supports a remote work setting, allowing our Licensed Clinical Social Worker to provide compassionate support from the comfort of their own home while leveraging telehealth technologies. However, there will be a blend of clinical interaction taking place as you coordinate with clients either virtually or in various community settings, ensuring they receive holistic and responsive care. The work facility is primarily focused on hospice care, which requires a deep understanding of end-of-life issues and sensitivity towards patients and their families.

Key Responsibilities\n

• Engage directly with clients who are experiencing severe emotional disorders and symptoms of major mental illnesses, assessing their needs through evidence-based approaches.

• Develop, implement, and monitor individualized care plans that encompass therapy goals, treatment options, and resources, ensuring a seamless exchange of information among clients, families, and healthcare providers.

• Conduct thorough assessments and screenings, and when necessary, arrange for additional evaluations to address complex client needs.

• Participate in crisis intervention efforts, utilizing appropriate tools and strategies to provide immediate support and solutions.

• Work collaboratively as a member of the healthcare team, contributing to interdisciplinary discussions aimed at enhancing patient outcomes and care quality.

• Ensure timely documentation and reporting of client progress, treatment efficacy, and critical incidents aligning with organizational standards.

• Provide oversight of case management functions, including client meetings and adherence to admission and discharge criteria.

Qualifications\n

• A valid Licensed Clinical Social Worker (LCSW) license in Arizona.

Preferred Experience\n

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
