Key Responsibilities\n

• Conduct comprehensive assessments and develop tailored treatment plans to meet the unique needs of clients.

• Facilitate pre-crisis and crisis management planning, which includes creating behavioral guidelines and positive behavior support strategies.
\n
• Ensure timely and accurate documentation in compliance with state regulations and organizational standards.
\n
• Employ evidence-based screening tools as outlined by our clinical team to better inform treatment decisions.
\n
• Collaborate with multidisciplinary teams to promote effective program implementation and client progression.
\n
• Actively engage in licensing and accreditation processes to maintain the highest standards of care.
\n
• Serve as a representative and liaison between clients and service systems, state agencies, and community organizations to foster collaboration.
\n
• Maintain essential open communication channels with stakeholders to uphold our quality service delivery and community engagement.
\n
• Participate in meetings, task forces, and training sessions to continue your professional development and to contribute to ongoing organizational improvement.

• Promote Transformative Unicare’s mission and values through all interactions and work activities.

• Perform other duties as assigned by the Clinic Director to support the overall goals of the organization.

Patient Population / Client Focus\n

You will work with a diverse population, including individuals facing mental health challenges, substance use disorders, and those with developmental disabilities or acquired brain injuries. Your role will be critical in helping clients navigate their circumstances, offering both therapeutic interventions and supportive guidance while fostering their autonomy and community integration.

Qualifications\n

• Master’s Degree in a behavioral health field is required.

• Holds an independent LMHC license issued by the relevant state board, ensuring compliance with state regulations.

• Proven ability to maintain accurate clinical records and prepare reports as necessary.

• Strong collaborative skills to work effectively within an interdisciplinary team setting.

• Excellent verbal and written communication skills for effective expression of ideas and treatment rationales.

Preferred Experience\n
