Medical Case Manager Job Description Template
Our company is looking for a Medical Case Manager to join our team.
Responsibilities:
- Ensure continuity of care by acting as a liaison between patient’s primary care / medical home and Hazel Health Providers;
- Implements care plan, facilitates referrals, coordinates services and resources and provides ongoing monitoring and re-evaluation of outcomes;
- Manage patient case assignments, draft treatment plans, review case progress and determine case closure;
- Provide back-up coverage to School Support, Spanish medical interpretation, and other Clinical Support as necessary;
- Other duties as assigned;
- Play an integral part of coordinating information between providers and students and their families;
- Coordinate and provide case management that is timely, effective, efficient, equitable, and student-centered;
- Additional duties as required;
- Utilizes their medical and nursing knowledge to discuss the current treatment plan with the physician and discuss alternate treatment plans;
- Participation in professional associations keeps the case manager informed of events in their field while establishing referral contacts;
- Heavy travel required; valid driver’s license and good driving record with no traffic violations required;
- Requires regular and consistent attendance;
- Develops Independent Medical Evaluation Plans. Provides assessment, planning, implementation and evaluation of patient’s progress;
- Coordinates injured workers’ appointments and arranges and/or personally escorts him/her to the appointments;
- Monitors functions assigned to non-case managers and provides input on the performance of support staff to their supervisor.
Requirements:
- Ability to work as a team player and adapt to changing workload and circumstances effectively;
- Ability to maintain confidentiality, exercise discretion, work independently and commit to excellence;
- Organizational skills to prioritize workload, flexibility and detail oriented;
- Strong verbal and written communication skills, and the demonstrated ability to understand and convey information clearly;
- Certification as a Case Manager;
- Reliable transportation and local travel required;
- Minimum 2 years experience in a case management role, preferably in a clinical setting;
- Worker’s Compensation or Case Management experience preferred;
- Able to work independently, be proactive, and assertive;
- Bilingual- Spanish speaking;
- All candidates must be eligible to work in the United States without VISA sponsorship;
- Ability to drive, maneuver, navigate on a continual or almost continual basis;
- Excellent written and verbal communication skills;
- Ability to meet designated deadlines;
- Computer proficiency and technical aptitude with the ability to utilize MS Office including Excel spreadsheets.
