Our company is looking for a Therapist to join our team.

Responsibilities:

  • Provides client casework services to address clinical social service needs;
  • Provides crisis intervention services in partnership with Glencoe Public Safety;
  • Works collaboratively with a range of community partners and FSG colleagues to plan and deliver education programs and events;
  • Completes intake assessments, treatment plans, progress notes, and discharge summaries;
  • Assist clients through transition process and complete final correspondence such as transition summary, plan, letter, etc;
  • Diagnosing patient substance use, mental and emotional disorders;
  • Assisting patients through difficult life changes and challenges;
  • Helping patients discuss experiences and emotions;
  • Working and clinical groups, communities, and individual patients;
  • Timely completion of FASP reports;
  • participate in treatment team meetings;
  • Provide measurement-informed care and utilize standardized assessment measures to gather both clinical information and therapy outcome data;
  • Formulates appropriate treatment plan along with consumer;
  • Provide at least once weekly home , office and/or community based services to families;
  • Schedule Family and Elevated Risk Conferences as needed.

Requirements:

  • Good communication and interpersonal skills;
  • Able to work nights and weekends if needed;
  • Ability to work with different insurance companies and note appropriately to obtain authorization for services rendered/needed;
  • Comfortable with building personal relationships and dedicated to helping others;
  • Self-motivated and self-directed;
  • Excellent interpersonal skills with patients, colleagues, and community resources;
  • Calm and cool-headed with a positive attitude;
  • Excellent time management skills; organized and able to prioritize;
  • Licensed or license-eligible preferred;
  • Experience working with adolescent and families;
  • Experience providing therapy in a community setting, particularly within homes and schools;
  • Ability to effectively collaborate with multidisciplinary teams to determine appropriate treatment and support plans;
  • Must have a valid Illinois driver’s license, reliable means of transportation and maintain safe driving record;
  • Experience providing psychoeducation and consultation to helping professionals and parents/caregivers;
  • Must have confidence working independently while also prioritizing collaboration with client and family support teams.
