Our company is looking for a Care Manager to join our team.

Responsibilities:

  • Provides training and updates on EMR usage and workflow;
  • Manage care Transitions from ER, Hospital and Nursing Home, coordinated with primary care provider staff and hospital staff;
  • Coordination of Clinical Quality Improvement (CQI) Activities;
  • Plans, directs, and evaluates the overall services and functions of the care coordination resource center (CCRC);
  • Coordinates quality, service excellence and performance improvement initiatives;
  • Ensures departmental compliance with regulatory agency requirements;
  • Supervises staff, evaluates work and implements approved disciplinary action. Develops staff to expert level of practice;
  • Monitors and adheres to annual hospital operating budget.

Requirements:

  • Bilingual is a plus;
  • Experience with Bridges to Health (B2H) a plus;
  • Bachelor of Arts/Science with two years of experience OR;
  • MBA, MSW, or MHA degree;
  • Ability to take call remotely on some nights and weekends;
  • Master’s Degree required;
  • Excellent verbal and written skills;
  • 2+ years of direct practice experience in settings that include inpatient, partial, and/or outpatient preferred;
  • Ability to show empathy and quickly build relationships with patients and physicians;
  • Active DC clinical licensure;
  • Occasional domestic travel may be required, primarily to Nashville, TN;
  • MSN degree;
  • Self-starter with the ability to work independently with minimal supervision;
  • Current valid and unrestricted medical license in the State of Maryland (LPN, RN);
  • Ability to learn new mobile phone and web-based applications.
