Job Description The Occupational Therapist-Home Health in northern Scottsdale plays a vital role in delivering personalized care and rehabilitation services to primarily adult patients within their homes. Working as part of an interdisciplinary team, this therapist conducts assessments, develops tailored care plans, implements therapy, and provides patient education to enhance patients' functional independence. Frequent travel to multiple homes is required to ensure comprehensive therapy delivery and ongoing progress monitoring.

Perform regular home visits, adapting therapy based on patient progress and environment
Monitor and document patient progress using electronic health records
Minimum 1 year of experience in occupational therapy practice
Licensed Occupational Therapist (OT)
Proficient in patient assessment, care planning, and therapeutic techniques
Experience in home health care delivery
Skilled in documentation using electronic health records systems
