Role Overview:
A Physical Therapist provides one-on-one geriatric care in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment and evaluations.
Responsibilities:
Deliver personalized therapy sessions supporting patient wellness and functional independence
Develop and implement individualized care plans
Document care effectively within EMR systems
Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
Educate patients, caregivers, and staff on safety and aging in place
Incorporate telehealth and virtual care to extend access and flexibility
Qualifications & Skills:
Valid state Physical Therapy license or eligibility
Degree from an accredited program
Proficiency in computer and EMR systems
Current CPR certification
Physical & Other Requirements:
Ability to exert medium physical effort, lift up to 50 lbs, and assist with patient transfers
Repositioning, standing, walking, and climbing stairs
Driving to patient locations as part of care delivery
Wearing PPE when necessary
