This Home Health Physical Therapist opportunity offers the chance to practice in beautiful Volusia County, Florida, a region known for its vibrant coastal lifestyle and welcoming communities. You will manage a diverse caseload that may include post-surgical orthopedic patients, individuals recovering from stroke or neurologic conditions, chronic disease management cases, balance and fall risk patients, and those requiring strengthening, endurance, and functional mobility training. This position offers a high level of autonomy and the opportunity to form meaningful therapeutic relationships while helping patients remain safely in their homes.You will coordinate closely with referring providers, nursing, occupational therapy, and other disciplines to support optimal outcomes and readmission prevention. Daily documentation is completed via electronic systems, with an emphasis on accuracy and timeliness to support quality and regulatory compliance.The typical day involves traveling within an assigned territory in Volusia County to complete scheduled patient visits, prioritizing efficiency while maintaining strong clinical care. Visit frequency and caseload are structured to allow focused, quality time with each patient while supporting reasonable daily productivity expectations. This position is ideal for a Physical Therapist who enjoys independence, community-based care, and the ability to practice at the top of their license. Working in Volusia County allows you to serve a diverse population while enjoying the amenities of a coastal Florida community, including beaches, outdoor festivals, motorsports heritage, and a variety of dining and cultural experiences. Shifts and hours are typically structured around daytime, weekday schedules, with some flexibility to meet patient needs. Weekend or occasional evening visits may be necessary depending on caseload and agency requirements, but the home health setting generally allows for collaborative scheduling and planning. Travel is localized within Volusia County, limiting long commutes and allowing you to get to know the communities you serve.

Physical Therapist, Home Health Licenses: PT-FL License and HH experience are required.
