Physical Therapist (PT) - Home Health

Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team. The Physical Therapist will provide therapeutic services to patients in their homes, helping them improve mobility, restore function, manage pain, and achieve the highest possible level of independence and quality of life.

Essential Responsibilities

Perform comprehensive physical therapy evaluations and assessments.

Develop and implement individualized treatment plans based on patient needs and physician orders.

Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.

Educate patients, families, and caregivers on treatment plans, exercise programs, safety measures, and injury prevention.

Assess and recommend appropriate assistive devices, equipment, and home safety modifications.

Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.

Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare professionals to coordinate patient care.

Maintain accurate, timely, and compliant clinical documentation in the electronic medical record.

Participate in interdisciplinary care planning and case conferences.

Ensure compliance with all federal, state, agency, and professional practice standards.

Qualifications

Current Physical Therapist (PT) license in the state of practice.

Graduate of an accredited Physical Therapy program.

Current CPR/BLS certification.

Home Health experience preferred but not required.

Strong clinical assessment, communication, and organizational skills.

Ability to work independently and effectively manage patient schedules.

Valid driver's license, reliable transportation, and proof of auto insurance.
