Description
Assess patient needs in both inpatient and outpatient settings while crafting personalized treatment plans
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Engage collaboratively with nurses, physicians, case managers, and rehabilitation professionals to achieve optimal patient outcomes
Educate patients and their families about therapeutic goals, discharge plans, and home exercise programs
Document evaluations, treatments, and patient progress in compliance with facility and regulatory standards
Participate actively in care team rounds and interdisciplinary meetings to ensure seamless patient care
Education
~ Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications
Valid state licensure as a Physical Therapist
Preferred: CPR certification
Specialty certifications (neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience in the field is strongly preferred

Exceptional communication and collaboration skills
