Da Vinci experience is a huge plus but not required.
Start- ASAP - Washington state license must be in hand at time of present. No IMLC at this time. We are going to seek fast track or urgent credentialing.

Procedures- standard urology procedures in the clinic and operating room
Required clinical activity in the last 24 months case logs- Applicant must provide documentation case list of provision of surgical services 100 procedures representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed.
Will provider work solo? If not other providers specialties working with them- Yes
Licensed Candidate: now open to seeing candidates with an IMLC but they must have the letter of qualification in hand as well. It is our understanding that with an LOQ a Washington license can be issued in less than week for MD and about 3.5 weeks for DO.
Required certifications Must be from American Heart Association or American Red Cross as applicable- BLS and ACLS

• **Admin Time is paid at an hourly rate
• ** Call is restricted/Beeper with a 10' return call response time a radius of 30 mins might be 45
• **Average After hour text/call 0-4
