At Active Motion, we are committed to excellence in treatment and fostering a collaborative working environment.Work EnvironmentThis position will be conducted onsite at our Woodbridge rehabilitation center, where you will collaborate closely with a team of two experienced physical therapists and one physical therapy assistant. The practice operates Monday through Friday, from 9 AM to 5 PM, allowing for a structured work schedule that promotes work-life balance. Our facility is designed to encourage teamwork and innovation, enabling each team member to contribute meaningfully to patient care.Key ResponsibilitiesConduct thorough assessments of patients to establish treatment goals and tailor individualized care plans based on their specific needs.Administer one-on-one patient treatment sessions, managing a typical caseload of one patient per hour to ensure focused attention and care.Utilize diverse treatment techniques and modalities, including manual therapy, therapeutic exercise regimens, and functional training, to facilitate recovery and improvement.Regularly monitor and document each patient's progress, adapting treatment plans as necessary to enhance outcomes and effectiveness.Educate patients about their conditions, anticipated treatment outcomes, and strategies for at-home care and wellness to empower them toward self-management.Foster open communication with patients about their progress, addressing any concerns and celebrating achievements to keep them motivated throughout their rehabilitation journey.Work collaboratively with other healthcare professionals, including physicians and specialty providers, to optimize integrated patient care.Participate in the development and implementation of fitness and wellness programs that target specific patient populations for ongoing health and vitality.Patient Population / Client FocusAt Active Motion Physical Therapy Services, our primary focus is on orthopedic patients requiring outpatient rehabilitation. This includes individuals recovering from sports injuries, post-surgical rehabilitation, and those with chronic pain conditions.
