Motion Back Physical Therapy in Carteret, New Jersey, is seeking a dedicated and skilled Physical Therapist to join our team. In this full-time role, you will provide exceptional care to patients within the home care setting, delivering treatments that enhance recovery and improve their quality of life. Work Environment

This position is onsite and involves direct patient interaction within the comfort of their homes, allowing for personalized therapy sessions that cater to individual needs. Working Monday through Friday from 9 AM to 5 PM, the role provides a balanced work schedule with ample opportunity to build meaningful relationships with patients and their families while making a significant impact on their recovery process.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical capabilities and needs to formulate effective treatment plans in collaboration with physicians and the care team.
• Implement individualized therapy regimens focusing on achieving physical rehabilitation goals and enhancing patient engagement throughout their treatment process.
• Deliver physical therapy interventions directly in patients' homes, utilizing various techniques to promote optimal recovery aligned with prescribed plans of care.
• Educate patients, families, and caregivers on therapeutic exercises, the use of adaptive devices, and best practices for at-home care to ensure continuity of treatment outside therapy sessions.
• Monitor patient progress, adapt treatment plans as necessary, and document all interventions and outcomes in accordance with professional regulatory standards.
• Prioritize caseload management and coordinate with colleagues for efficient and effective patient transitions.

Patient Population / Client Focus

You will be working with a diverse patient population, including individuals recovering from surgery, chronic pain, neurological conditions, and other mobility impairments. Your expertise will be vital in providing tailored care that addresses the unique needs of each patient, ultimately contributing to their overall health and lifestyle improvements.

Qualifications

• A Bachelor's Degree in Physical Therapy (Doctor of Physical Therapy preferred), demonstrating your commitment to high standards in patient care.
• State licensure as a Physical Therapist, ensuring adherence to legal and ethical guidelines in practice.
• A clean disciplinary record reflecting your professional integrity and commitment to ethical standards.
• Strong manual therapy skills, allowing you to deliver precise and effective treatment.

• Familiarity with electronic medical record (EMR) systems such as WebPT, supporting efficient documentation and information sharing within the healthcare team.

Preferred Experience

• Experience in home care settings, understanding the specific dynamics and challenges of providing therapy in patients' residences.
• Background in pediatrics or geriatrics, enhancing your ability to engage with various age groups effectively.

Compensation and Benefits
