Autonomous RN Case Manager - Home Hospice and In-Home Care

ProHealth Care is looking for an RN Case Manager to work collaboratively with patients and healthcare providers in coordinating patient care. This position emphasizes independent work, critical thinking, and effective communication while managing home care for patients. The RN will assess, plan, and facilitate care, providing hands-on assistance in a flexible environment.
