Work Environment

The successful candidate will work directly within clients' homes, providing tailored physical therapy services and assessments to promote rehabilitation. This role requires collaboration with interdisciplinary team members, including physicians, nurses, and other healthcare specialists, to ensure optimal care delivery. Our workweek is structured Monday through Friday, allowing dedicated time for both patient interaction and professional development.

Key Responsibilities

• Direct physical therapy treatment aimed at reducing pain, preventing disability, and enhancing muscle function through personalized care plans.
• Evaluate patients' functional capabilities and home environments to develop actionable rehabilitation strategies that improve independence.
• Direct the activities of Physical Therapy Assistants to ensure compliance with care plans and effective service delivery.
• Conduct thorough assessments, utilizing diagnostic tests for muscles, nerves, and joints to design evidence-based treatment protocols.
• Supply prescribed physical therapy services, including manual and non-manual exercises, muscle strengthening, and the application of therapeutic modalities such as traction and massage.
• Monitor patient progress, documenting changes and responses to therapy while providing insightful feedback to the care team.
• Participate actively in the development and update of care plans, attending interdisciplinary meetings to contribute to holistic patient care.
• Ensure compliance with all documentation requirements, including clinical records, evaluations, and progress notes, submitted timely as per branch policies.
• Instruct clients and families on rehabilitation techniques and usage of therapeutic devices to foster self-management and independence.
• Complete functional assessments on-site and adjust care plans dynamically based on ongoing evaluations and client feedback.
• Remain available for consultation and support of Physical Therapy Assistants during therapy sessions as needed.
• Carry out additional assignments to further the mission and objectives of Decker Physical Therapy Center.

Patient Population / Client Focus

The Physical Therapist will work with a diverse patient population, including individuals recovering from orthopedic surgeries, those managing chronic pain issues, and clients requiring rehabilitation after injuries. Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program is mandatory.
• Current state licensure as a Doctor of Physical Therapy (DPT) is required.
• A minimum of two years of relevant clinical experience in a physical therapy setting, preferably within home healthcare or outpatient services.
• A current CPR certification is essential for maintaining client safety.

Preferred Experience

• Experience working with diverse patient populations, particularly in home health or community settings, is highly valued.
• Familiarity with documentation requirements related to Medicare and other insurance providers enhances the skill set.
• Previous experience supervising or mentoring Physical Therapy Assistants is a plus.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
