Key Responsibilities

• Conduct thorough evaluations for patients, including comprehensive history reviews and physical assessments to develop individualized treatment plans.
• Articulate evaluation findings, treatment plans, and expected outcomes clearly to patients and their families, ensuring their active involvement in the goal-setting process.
• Implement evidence-based treatment protocols that are specifically tailored to each patient's assessment results and ongoing health status.
• Collaborate with other healthcare professionals as necessary to address complex patient issues and to document any changes in a patient's condition that require adjustment to their care plan.
• Oversee discharge planning and provide follow-up care recommendations, ensuring timely and accurate documentation of patient progress, treatment encounters, and discharge evaluations.
• Exhibit excellent time management skills, adhering to productivity expectations while maintaining an organized schedule for appointments and meetings.
• Actively participate in hospital and community marketing activities to promote the clinic's services and foster community relationships.
• Contribute to achieving departmental objectives and support compliance with all regulatory and accreditation standards.
• Pursue continuing education opportunities, enhancing both personal and organizational competencies, while mentoring students or new hires as a clinical instructor.

Patient Population / Client Focus

The Physical Therapist at Pacific Physical Therapy Clinic will work primarily with adult and senior patients who require rehabilitation services following injury, surgery, or chronic conditions. Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution is essential.
