Key Responsibilities\n

• Conduct thorough biopsychosocial assessments to identify the mental health, substance abuse, and medical needs of patients in a hospice care context.

• Document all mental health evaluations, progress notes, individual and group therapy notes electronically in our EMR system to ensure accurate and timely information for care coordination.
\n
• Offer continuous behavioral health services, providing counseling and support to individuals, couples, and families navigating complex grief and anticipatory loss.
\n
• Develop treatment plans tailored to the needs of the patients, including crisis intervention and referrals to community resources, ensuring a continuum of care and support.
\n
• Work collaboratively with interdisciplinary teams, including physicians, nursing staff, and adjunct wellness professionals, to refine processes and implement data collection strategies for quality improvement.

• Act as a liaison between patients and community resources, including but not limited to, medication assistance programs and local mental health services.

Patient Population / Client Focus\n

This position primarily serves patients in hospice care who often face not only terminal illnesses but also the psychological complexities associated with end-of-life issues. You will work with a diverse client base, needing specialized support for grief, depression, anxiety, and related conditions, often requiring you to demonstrate high emotional intelligence and adaptability in your therapeutic approaches.

Qualifications\n

• A Master’s degree in Social Work (MSW) is required.

• Current and valid Licensed Clinical Social Worker (LCSW) license in Pennsylvania is mandatory.

• Three (3) years of post-licensure experience in mental health settings, emphasizing the importance of therapeutic techniques and independent judgment in service delivery.

• Expertise in crisis intervention and a solid grounding in various counseling methodologies.
