Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be responsible for delivering tailored clinical behavioral health services that align with CHDC's mission and quality standards. Your contributions will focus on enhancing patient health outcomes and satisfaction through the following key responsibilities:

• Guide mental health consumers in navigating both physical and behavioral healthcare systems to create a cohesive and integrated health experience.

• Educate patients about available resources and services that promote disease prevention, wellness, and a healthy lifestyle, thereby contributing to overall community health.

• Participate actively in interdisciplinary treatment team meetings, offering insights and collaborating on integrated care strategies.

• Ensure prompt reporting of crises and unusual incidents to the Medical Director and communicate psychiatric or medical emergencies immediately to the Chief Medical Officer (CMO).

• Maintain meticulous clinical documentation for individual patient records, ensuring compliance with all relevant health regulations and standards.

• Engage fully in the Quality Improvement programs by attending meetings, training sessions, and implementing learned practices.

• Collaborate with the team in utilizing evidence-based screening tools to assess and address the healthcare needs of individuals.

• Provide specialized support for patients dealing with Opioid Use Disorder in partnership with our Center of Excellence Team, aiming to deliver comprehensive recovery-oriented services.

In this role, you will work closely with a diverse patient demographic, including individuals facing complex medical and psychological needs. Your efforts will directly impact those receiving hospice care, ensuring that they and their families receive the emotional and psychological support necessary during critical transitional times. Qualifications\n

A successful candidate for this position must possess the following qualifications:

• A master’s degree in social work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.

• At least 2-3 years of relevant experience in clinical social work or related fields, particularly in a healthcare setting.
