Key Responsibilities\n

• Conduct thorough assessments and develop individualized treatment plans that address the specific therapeutic goals of clients and their families.

• Document all client interactions and treatment activities in a timely and accurate manner to maintain compliance with healthcare regulations.

• Participate in ongoing training sessions, clinical meetings, and quality assurance activities to stay updated on best practices and enhance service delivery.

• Collaborate with a multidisciplinary team, providing insights and professional guidance to other healthcare providers regarding patient treatment.

Patient Population / Client Focus\n

The Licensed Clinical Social Worker will primarily work with patients in hospice care, who may be facing terminal illnesses and related emotional challenges. Qualifications\n
