Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients' mental health status, strengths, and areas requiring support.

• Provide therapeutic interventions through individual, family, and group therapy sessions utilizing evidence-based practices, such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Develop and implement individualized treatment plans based on comprehensive assessments, setting measurable goals and employing appropriate interventions.

• Collaborate with psychiatrists, psychologists, and multidisciplinary team members to ensure coordinated and comprehensive care for each client.

• Offer crisis intervention services, including risk assessments and the formulation of safety plans to address acute emotional distress.

• Facilitate supportive group therapy sessions, psychoeducational workshops, and family support groups to enhance community resources for patients.

• Maintain meticulous documentation in compliance with ethical guidelines and legal standards, ensuring efficient record-keeping and communication.

Patient Population / Client Focus\n

Your client focus will primarily include individuals in hospice care, their families, and caregivers. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
