Transitional Care Unit Registered Nurse - $32-48 per hour

Job Summary Registered Nurse (RN) – Transitional Care Unit (TCU) – Ongoing staff position at BJC HealthCare, Saint Louis, Missouri. Shifts & Schedule 3x12 hour shifts Every 3rd weekend Night shifts Sign-on Bonus up to $15k eligible for external applicants Responsibilities Provide direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation in accordance with nursing standards. Promote patient and family centered care in a healing environment. Educate patients and families on illness management, post-treatment home care needs, and medication administration. Participate in patient safety, quality, and regulatory compliance activities. Engage in professional development activities. Develop, implement, and document individualized plans of care with defined goals in collaboration with the interprofessional team, patients, families or caregivers. Proactively plan and ensure communication of the plan of care across the continuum of care. Use critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to the standards of care. Assess patient preferences and barriers to care, including values, emotional, spiritual, cultural, and population-specific needs. Maintain constant state of alertness and work safely as a safety-sensitive position.
